    Pandemic Influenza

    An influenza pandemic is an epidemic of the influenza virus that spreads on a worldwide scale and infects a large proportion of the human population.

    In contrast to the regular seasonal epidemics of influenza, these pandemics occur irregularly, with the 1918 Spanish flu the most serious pandemic in recent history.

    Pandemics can cause high levels of mortality, with the Spanish influenza being responsible for the deaths of over 50 million people.

    There have been about 3 influenza pandemics in each century for the last 300 years. The most recent ones were the Asian Flu in 1957 and the Hong Kong Flu in 1968.

    Seasonal Influenza

    Flu season is the portion of the year in which there is a regular outbreak in flu cases.

    It occurs during the cold half of the year in each hemisphere.

    Flu activity can sometimes be predicted and even tracked geographically. While the beginning of major flu activity in each season varies by location, in any specific location these minor epidemics usually take about 3 weeks to peak and another 3 weeks to significantly diminish.

    Individual cases of the flu however, usually only last a few days. In some countries such as Japan and China, infected persons sometimes wear a surgical mask out of respect for others.

    Avian (Bird) Flu
    Avian influenza,

    sometimes Avian flu, and commonly Bird flu refers to "influenza caused by viruses adapted to birds."

    "Bird flu" is a phrase similar to "Swine flu", "Dog flu", "Horse flu", or "Human flu" in that it refers to an illness caused by any of many different strains of influenza viruses that have adapted to a specific host.

    All known viruses that cause influenza in birds belong to the species: Influenza A virus.

    All subtypes (but not all strains of all subtypes) of Influenza A virus are adapted to birds, which is why for many purposes avian flu virus is the Influenza A virus (note that the "A" does not stand for "avian").
    Adaptation is non-exclusive.

    Being adapted towards a particular species does not preclude adaptations, or partial adaptations, towards infecting different species.

    In this way strains of influenza viruses are adapted to multiple species, though may be preferential towards a particular host.

    For example, viruses responsible for influenza pandemics are adapted to both humans and birds.

    Recent influenza research into the genes of the Spanish Flu virus shows it to have genes adapted to both birds and humans; with more of its genes from birds than less deadly later pandemic strains.

    H5N1 Strain

    Influenza A virus subtype H5N1, also known as A(H5N1) or simply H5N1, is a subtype of the Influenza A virus which can cause illness in humans and many other animal species.

    A bird-adapted strain of H5N1, called HPAI A(H5N1) for "highly pathogenic avian influenza virus of type A of subtype H5N1", is the causative agent of H5N1 flu, commonly known as "avian influenza" or "bird flu".

    It is enzootic in many bird populations, especially in Southeast Asia. One strain of HPAI A(H5N1) is spreading globally after first appearing in Asia.

    It is epizootic (an epidemic in nonhumans) and panzootic (affecting animals of many species, especially over a wide area), killing tens of millions of birds and spurring the culling of hundreds of millions of others to stem its spread.

    Most references to "bird flu" and H5N1 in the popular media refer to this strain.

    As of the July 25, 2008 FAO Avian Influenza Disease Emergency Situation Update, H5N1 pathogenicity is continuing to gradually rise in wild birds in endemic areas but the avian influenza disease situation in farmed birds is being held in check by vaccination.

    Eleven outbreaks of H5N1 were reported worldwide in June 2008 in five countries (China, Egypt, Indonesia, Pakistan and Vietnam) compared to 65 outbreaks in June 2006 and 55 in June 2007.

    The "global HPAI situation can be said to have improved markedly in the first half of 2008 [but] cases of HPAI are still underestimated and underreported in many countries because of limitations in country disease surveillance systems".

    Pandemic Severity Index

    The Pandemic Severity Index (PSI) is a proposed classification scale for reporting the severity of influenza pandemics in the United States.

    The PSI was accompanied by a set of guidelines intended to help communicate appropriate actions for communities to follow in potential pandemic situations. [1]

    Released by the United States Department of Health and Human Services (HHS) on February 1, 2007, the PSI was designed to resemble the Saffir-Simpson Hurricane Scale

Statement of Interest: Prepared Citizens LLC exists to attempt to inform and educate the residents of Monson, through the encouragement, support and validation of the Town of Monson, of the need to prepare for a pandemic of influenza. We enlist and encourage the assistance of fellow citizens and other civic groups for the dissemination of pandemic preparedness information. 

Organization of town resources will be necessary in order to accomplish this. By fostering healthy and caring relationships in the community there exists the hope of alleviating hunger, relieving isolation and fear and building post-pandemic resiliency. The goal is to create an atmosphere that allows for education and information sharing. Also to create an environment that encourages individuals and families to adopt changes that will help them to safeguard themselves during a health disaster.

Statement of Intent: As a ‘Pandemic Preparedness Activist’ my intent would be to increase the awareness of pandemic influenza and its effects on the citizens of Monson MA. Traditional media is a primary source of information during a crisis but during the preparedness stage the main source of information has become the internet. A wealth of information is available to those who diligently search for it but most people do not have the time for this, the knowledge that the need exists, and many do not have basic access to the information. Without this information residents will be caught off guard without the needed time to prepare.

It is my intent to utilize traditional and non-traditional methods of information dissemination in order to widen the knowledge base regarding this issue. A preparedness series in the local newspaper, information dissemination through town mailings, utilizing local public access television, information dissemination through schools, local business and the free library are all satisfactory and low cost methods of increasing awareness.

Goal 1: Increase access to information. Provide educational material to town residents so that they may make their own preparedness decisions.

Assumption: The town of Monson does not bear a responsibility during non-pandemic times for the maintenance of any of its citizens; likewise the town should not bear this burden during a pandemic. This would be an impossible task.

Any and all aid that may be given during a crisis such as a pandemic would gladly be given if available, as we are able, and would not come about because of any obligation to the citizens of the town. That being said, there is an understanding that some of our neighbors will face additional and perhaps insurmountable burdens during a pandemic and every effort will be made to assist them when resources to do so are available.

Plan of Action: There is a need to identify the resources in town, identify stakeholders and encourage volunteerism, cultivate solutions, and build a bridge between local government and town residents. It is my hope to weave a thread of cohesiveness through the town before, during, and after a pandemic.

Many challenges are ahead and many solutions will be needed. Working together to prevent disease will promote resiliency through action.

Objective 1: Identify those things that we are not doing now that may increase infection risk to the residents of the town. Educate regarding non-pharmaceutical interventions that are both practicable and effective such as social distancing, early school closure, frequent and thorough hand washing techniques, voluntary isolation and quarantine and other suggested interventions. Work with the town departments heads to ensure that continuity of operations plans are in place. Work with local health officers to ensure that currently in place regulations are being enforced. Work with the school services director to encourage closer adherence to infection control methods within the schools. Work with citizens on an ongoing basis to increase awareness.

Objective 2: Encourage the establishment of a town wide public health surveillance method for tracking illness in the hopes of reducing morbidity and mortality through early outbreak detection. Data collection from the state sentinels and school health professionals would have to be reviewed daily in order to track illness among the children and educators in town, businesses could also be included in this model as well as voluntary inclusion by residents. Follow up investigation of any case reports would also be necessary. This will serve to increase visibility of town health officers and increase educational opportunities. This will also identify at risk individuals within our community. Case follow-up utilizing the MRC and/or the health officers would have to become routine. Identifying the outbreaks in town will serve as a trigger for early school closure, acting in accordance with Massachusetts Department of Education regulations.

Success of this would depend upon the cooperation of town residents and employees. This cooperation would be dependent upon the increased awareness level outlined in Goal 1.

I would work toward establishing a call in number at the Board of Health office where employees and town residents may phone in to report influenza like illnesses as an attempt to establish this proper sentinel system in town. Calling in to the Board of Health will also ensure that privacy is safeguarded. This will serve to build a relationship between town residents and the Board of Health who will then become a trusted source for information dissemination during the pandemic, post wave, and post pandemic. It is vital that our Board of Health is understood to be a capable partner in helping to safeguard the public health of the residents of Monson.

Goal 2: Establish a town pandemic pre-response team

As pandemic change agent I would act as a “coach” of a team of individuals who are ready to step forward in order to accomplish the goals that we set forth. Among them I would suggest:

Suggestion A: Identify town wide issues and encourage novel solutions. Increase communication among the town residents and bring into crystal clarity the fact that will be at once “on our own” yet an integrated whole.

As a team of mitigation specialists we would also encourage town residents to develop novel approaches to the dilemmas that we will face. Our town employees are best able to identify these issues, a good working relationship is crucial. We must be seen as partners and not adversaries and not competitors for funds and limited resources. An open dialog with persons outside of the official town hierarchy, yet approved by the town selectmen and board of health may be able to encourage a free and open dialog more readily than someone within the system itself. As a team we must be seen as bridge builders.

Suggestion B: Enlist the support of town civic organizations, faith based groups, youth groups, and private individuals. Challenge these organizations to become innovators of solutions regarding social issues. Enlist the financial assistance of local businesses in producing educational materials for dissemination. Seeking aid and assistance from the business sector is also an opportunity for increasing awareness to the issue.

Volunteerism will be as important as social distancing and merging the two will be a challenge during a pandemic. Once cohesive groups already in place are aware of the challenges ahead they can forge ahead in their own area of interest and expertise, but first they must become aware of the need. For example: the PTSA may be interested in working with the superintendent and educators in order to ensure education continue despite extended school closure. The faith based groups may be trusted community partners who can identify and address the issues of hunger and deprivation. Youth groups can educate peers about hand washing and hygiene strategies. Ideas: Neighborhood watch, phone trees and neighborhood support systems, community food bank are all possibilities.In other words, I encourage a ‘many hands approach’. Action fosters resiliency.

The possibilities are limitless.

Because this is the first time that we have been able to prepare for a pandemic, which may or may not occur, any steps that are taken should be viewed as a learning experience and not as a set procedure that must be followed. In other words, this is more of an art form than a skill at this point.

As pandemic mitigation specialist I will, above all, strive to do no harm.

Monson has proven to be a town of innovators. I look forward to working with the town officials, the residents and the many friends and acquaintances whom I have come to know over the years that I have been a resident of this town.

My testimony

For me, the truth is, I don’t fear what others believe. I fear what I believe. As a child I lost my innocence and security. My life’s experience was to have my foundations shaken and shattered over and over. On hindsight I see God’s loving hand protecting me.

I sure have asked a lot of questions in my life. Why God? Why allow heartbreak? Why strip people away from who and what they love? This is life.

The “Christians” in my life had let me down. My parents, my relatives, they did not protect me as a child. I was so angry. I realized that I was hating the way the world can be. Cruel. Unfeeling, we are all both subject to it and capable of it. I knew that I needed a clean heart. I wanted a clean heart. I did not want to be caught up in this world system. I didn’t want to be a part of “the fall”. I had always wondered what the garden must have been like…before. And I so much wanted to return there. I sought the Kingdom of God here on earth for a time. It could not be found. I know that it is not here on earth but certainly there must be something to bless us like the wandering Jews were blessed in the desert – with Manna from Heaven. We are spiritually wandering before we find faith.

I knew opening my heart was risking alienating everyone around me. Then I remembered Nicodemus coming to Jesus Christ in the night. I prayed. A couple of times. But it didn’t “take”. I was fooling myself. I was not truly desiring an open heart. My mind was open not my heart. I could not pass through the faith barrier that I knew was there.

Time went on. I lived the natural way. And then I finally ran out of myself. The struggle to be good and do right was tiring. It was how I wanted to live but I was tired and depressed. I had a hole in my soul. Being born again sounded good to me. My heart was open as well as my mind. I prayed because *I* had nothing left. I prayed with heart, mind and soul.

I knew that I was/am a sinner. That Jesus Christ was born of the Virgin Mary. Both fully man and fully God. He suffered and died on the cross at Calvary for my sins. He was resurrected after the third day in the tomb. He appeared first to his disciples and then to thousands. He lives today and is seated at the right hand of the Father and is coming again to judge the living and the dead. All of this was and is my belief but it was on this day and at this moment that I realized the truth of these words. I gave my life, heart and soul to Jesus Christ. And I have never, ever, regretted this decision in my life.

At the moment of first real belief, I felt a “thrill” pass through me. I KNEW. The Holy Spirit had been poured out on me, my heart was filled. I told my husband, my children and anyone who I thought may understand what I was experiencing. I had no previous knowledge of any of this. I opened my Bible and lo and behold I could really understand the words. I became insatiable for the Word of God. I had been born again.

I was baptized in fire and then the actual water baptism. Forever and thankfully changed.

I found healing from some things. Actual physical healing. Some absolute emotional healing, and some things have been left to keep me vigilant and thankful for what has been healed.

The passion….It’s an intensity thing. I do try to keep it pretty moderate because I know that I can be overwhelming. I have always been and intense individual. My walk is no different.

I hope that I am never silent about the wrongs that are taking place in the world. But my hope is in the promises of God.

It is true that we must “let our little light shine”.

We are in a period of time about which the Bible says more than it does about any other time in history. Understand and watch – do not be deceived. We know what is ahead based on the Word of God. We walk toward what is ahead with our eyes wide open wearing the full armour of God (Eph 6:10-17 ). Our hope is in the promises that God has made and our sustenance is in knowing that these promises will be kept.

“If I shut up the heavens so that there is no rain, or if I command the locust to devour the land, or if I send pestilence among My people, and My people who are called by My name humble themselves and pray and seek My face and turn from their wicked ways, then I will hear from heaven, will forgive their sin and will heal their land.” 2 Chronicles 7:14

Faith, Hope, and Love….

Faith – “The life which I now live in the flesh I live by faith in the Son of God.” Galatians 2:20

It is by faith in the gospel that we receive the favor of God which cannot be earned, we simply must believe in God’s Son who made Himself the sacrifice for humanity’s fallen state. This is a gift from God to His creation. The only way to God is through faith in His Son Jesus Christ.

Love – “For I am persuaded that neither death, nor life, nor angels, nor principalities, nor things present, nor things to come, nor powers, nor height, nor depth, nor any other created thing shall be able to separate us from the love of God, which is in Christ Jesus our Lord.” (Romans 8:38-39)

“No one has seen God at any time; if we love one another, God abides in us, and His love is perfected in us. By this we know that we abide in Him and He in us, because He has given us His Spirit.” 1 John 4:12-13.

Such a blessing He knew you were calling for Him from your heart..really wanting Him…in a way-He chases us, until we either finally turn like He so wants us to, or He lets us keep stumbling our way…He sure was awesome to me-no matter what illness, pain arised-it was all about Him and finding Him!
